Pier St Underpass is an accessible pedestrian underpass located near Darling Harbour, connecting Ultimo with the main Darling Harbour area. It is easy to reach by walking from Tumbalong Park or International Convention Centre Sydney, and it is also close to bus stops and light pedestrian routes in the CBD. The pathway has accessible paved area, generally step-free and suitable for wheelchairs and prams, making it a useful link between busy areas.

Inside the underpass, there is also a piano station, which adds a public art and interactive element to the space. There are unisex ambulant and unisex accessible toilets with automatic doors located on one side of the walkway, clearly signposted for public use. The space itself is simple and functional, with concrete walls and open walking space designed for continuous pedestrian movement. Along the sides, there are some seating benches and stone seating areas where people can rest briefly. The underpass is well-lit and practical rather than decorative, mainly serving as a safe and direct connection route between different parts of Darling Harbour.
